Reporting Loss and Replacement of Identity Card (IC)
Singaporean citizens who have lost their IC are required to report the loss and apply for a replacement in person within 14 days. A police report is required only if you are a victim of crime. Otherwise, you may execute a Statutory Declaration duly notarised by a Notary Public or when you go to ICA to report the loss.

Change of Name or Address in Identity Card (IC)
You may only change the name or address in your IC directly with the Immigration & Checkpoints Authority.
