Reporting Loss and Replacement of Identity Card (IC)
Application for a replacement can only be made personally at the Immigration & Checkpoints Authority. For full details and procedures on reporting loss of IC, please visit https://www.ica.gov.sg/
Singaporean citizens who have lost their IC are required to report the loss and apply for a replacement in person within 14 days. A police report is required only if you are a victim of crime. Otherwise, you may execute a Statutory Declaration duly notarised by a US Notary Public or when you go to ICA to report the loss. Upon returning to Singapore, please visit the Immigration & Checkpoints Authority to report the loss.
Change of Name or Address in Identity Card (IC)
