CONSULAR MATTERS - APPLICATION FOR A NEW SINGAPORE INTERNATIONAL PASSPORT
REQUIREMENTS:
- Your existing original Singapore Passport. IF your passport is lost, a copy of police report or a notarised Statutory Declaration Form will be required.
- For replacement of lost passport, please also print and complete the following form that is applicable. This is in addition to the required Passport Application Form and Additional Information Forms. :
- If the passport is damaged, please submit the damaged passport for cancellation.
Note: Your damaged or original existing passport will be cancelled and returned to you for your retention upon the issuance of a new passport.
- Paste one passport-sized photograph in the space provided on the top right hand corner of the form. The photograph must be in colour and shall be :
- Sharp and clear, 35mm wide by 45mm high without border and taken within the last 3 months;
- Taken full face with you looking directly at the camera with head straight and eyes open. Your hair should not be across and/or covering your eyes. Both edges of your face and the top of your shoulder must be clearly shown. You must not look over any shoulder and your head must not be tilted;
- Taken with the image of your face measuring between 25mm and 35mm from chin to crown of the head;
- Taken without wearing any hat or other head covering, unless you habitually wear a hat or head covering in accordance with your religious or racial custom. If any such hat or head covering is worn, the photograph must still be a full frontal view of your head and shoulders, showing your facial features in entirety with eyes open and clearly visible;
- If you wear glasses, the photograph must show your eyes clearly with no flash reflection off the glasses. The frames shall not cover any part of your eyes. Tinted glasses and sunglasses are not allowed;
- Taken with uniform lighting with no flash reflections, no shadows, no uneven bright spots on the face and no red eyes;
- Of white background, except that where if your hair, hat or head covering is white, the background must be light grey;
- The photograph must show you alone with no chair back, toys or other persons visible;
- The photograph must be printed on high quality paper at high resolution with a matt or semimatt finish with no ink marks or creases.
- Completed Passport Application Form for Singapore Biometric Passport Submitted Overseas -Form IMM (E) 11 (OS) . Only the 64-page passport will be issued. The validity period of Singapore passports issued on and after 1 April 2005 will be reduced from 10 years to 5 years. - Additional Information Forms (Form P-28.3) [To download the form, press "Ctrl" & click on the link simultaneously, or you may go to http://www.ica.gov.sg/data/resources/docs/Citizen%20Services/FormP28_3.pdf];
- Photocopy of National Registration Identity Card / Singapore Birth Certificate / Singapore Citizenship Certificate;
- If the applicant is a minor, please also submit a copy of parents' NRIC / Passport.
- Notarised/Certified True Copy of Marriage Certificate for insertion of married name, if any;
- Notarised/Certified True Copy of Baptism or Religious Certificate for insertion of religious name, if any;
- Notarised/Certified True Copy of Deed Poll for change of name, including ethnic name, if any;
- Copy of valid Exit Permit or Notice of Exemption for male applicants 13 years and above, if applicable.

The date of issue of your Singapore NRIC, a photograph image file in JPEG format (‘jpg’ extension) and a credit card for payment are required for online submission. The fee is SGD$70 and payment must be made by Visa or Mastercard credit card. Please indicate your contact information in the USA and select the respective Singapore Overseas Mission where you would like to collect your passport from.

NOTES:
Processing time is approximately 3 to 4 weeks from the date of receipt of the application. As the passport is issued in Singapore, please allow enough time for processing and possible delays in delivery.
