CONSULAR MATTERS - APPLICATION FOR CERTIFICATE OF DEATH EXTRACT

Please provide the following :

1. Completed application form;
   
   
2. A fee of US$23 for the Certificate (extract) is payable. Fee is subject to change without notice;
   
   
3. If you are applying this Certificate on behalf of relatives or friends, please forward a letter of authorisation indicating your name, your NRIC No and your relationship with the deceased. A copy of your NRIC should be attached. Reasons for the application should also be stated. If you do not have a Singapore NRIC, a certified copy of your passport must be submitted. Please also provide your local address and contact numbers;
   
   
4. Please furnish the correct Death Registration Number. If you do not know the Death Registration Number, please pay a search fee by filing a request for Search. The search fee is US$27 for every month searched. Fee is subject to change without notice.
   
   
5. Application may also be submitted online at http://www.ica.gov.sg .

1. The processing time is between 6 to 8 weeks.

To facilitate the delivery of your certificate by post, please provide a self-addressed and stamped United States Postal Service (USPS) Express Mail / Priority Mail envelope OR a self-addressed prepaid envelope from a courier service of your choice. In the latter, please make sure that pick-up service from our location is available.
