If your CPF withdrawal form requires the High Commission’s attestation, please call us to schedule an appointment. You must come personally and bring the following:

1. the CPF withdrawal form
   
   
2. your old and current passports (original copy)
   
   
3. identity card (original)
   
   
4. original bankbook or statement if you wish to credit the withdrawal to your bank account
