Application for an extract of Birth / Death Certificate

APPLICATION FOR BIRTH EXTRACT


A birth extract is a certified true copy of the Birth Certificate (BC). Once a birth certificate is lost and a replacement is required, applicant has to apply for a birth extract. All applications for a certified extract of these records have to be made on the requisite application forms. Applications can be made in person or via the Internet.

 

Eligibility

 

  • The BC holder or his parents.
  • Applications made on behalf of relatives or friends must be accompanied with a letter of authorisation stating the name and identification number of the applicant and his/her IC relationship with the BC holder. If the applicant is not holding a Singapore Identity Card, a certified copy of his/her official identification document (e.g. identity card, passport, driver's licence, etc) must also be submitted.

Procedure

 

Application in person

 

  • Submit application at:
    Citizen Services Centre
    Registry of Births & Deaths
    3rd Floor, ICA Building
    10 Kallang Road
    Singapore 208718

Application via Internet

 

  • Applicant must complete the on-line application form.
  • For Internet applications, please note that you are required to have an e-mail account anda VISA or Master Credit Card or Master Debit Card, for submitting your application via extracts online.
  • The processing time is approximately 1 week upon receipt of payment.

Documents Required

 

  • Applicant's Identity Card;
  • If the applicant is not holding a Singapore Identity Card, a certified copy of his/her official identification document (e.g. identity card, passport, driver's licence, etc) must also be submitted.

Fees (Effective 1st January 2013)

 

  • S$33.00 per birth extract

Note: If the Birth Registration Number is not furnished, applicants are required to pay a search fee:

 

  • For a month search period – S$38.00
  • For every additional one month search period or part thereof – S$38.00

All payments should be made in cheque/money order (for local applicants)/bank drafts (for overseas applicants) and made payable to the "Registrar of Births and Deaths" OR "Commissioner Immigration & Checkpoints Authority".

 

Payment for Internet Applications

 

Payment must be received within

  • 1 week from date of application for local applicants; or
  • 3 weeks from date of application for overseas applicants. The applications will be treated as cancelled and will be deleted from the system if it is not received by then.

Processing Time

 

 

Application in person

 

  • 1 week if the birth registration number is furnished. Otherwise, it will be longer as it involves additional effort in searching for the birth record.

Application via Internet

 

  • 1 week from the date payment is received only if birth registration number is furnished. Otherwise, it will be longer as it involves additional effort in searching for the birth record.

Collection

 

 

Overseas applicants

  • Receive the extract by post

Local applicants

  • Receive by post
  • Collection at our service counters.

Authorising someone to collect the certified birth extract (proxy)

  • Bring letter of authorisation stating the collector's name and identification number.
  • The collector's identification document (IC/passport) and the official payment receipt.
  • Without official payment receipt, the collector must furnish the application reference number.

For extracts sent by post, ICA will not be liable for any replacement should it arrive crumpled.



APPLICATION FOR DEATH EXTRACT

 

A death extract is a certified true copy of the Death Certificate (DC). Once a death certificate is lost and a replacement is required, applicant has to apply for a death extract. All applications for a certified extract of these records have to be made on the requisite application forms. Applications can be made in person or via the Internet.

 

Eligibility

 

  • The family members or anyone who is related to the deceased.

Procedure

Application in person


  • Submit application at:

Application via Internet

 

  • Applicant must complete the on-line application form.
  • Submit the form electronically and send payment to the Registry of Births & Deaths.
  • The processing time is approximately 1 week upon receipt of payment.

  • Applicant's Identity Card;
  • If the applicant is not holding a Singapore Identity Card, a certified copy of his/her official identification document (e.g. identity card, passport, driver's licence) must also be submitted.

Search Fees


If applicants are unable to provide correct Death Registration Number, a search fee is also required

  • Applications will only be processed upon receipt of the payment.
  • Approximately 1 week. If death registration number is not available, the processing time may be longer as additional effort is involved to search for the death records.

Collection


Overseas applicants


  • You will receive the certified death extract by post

Local applicants


  • You may choose to receive by post or by collection at our service counters.
  • For postal requests, ICA will not be liable for any replacement should it arrive crumpled.

Collection by Proxy


Applicants may authorise someone to collect the certified death extract on his/her behalf. The following must be produced:

 

  • Letter of authorisation stating the collector's name and identification document number.
  • Collector's identification document and official payment receipt.
  • Without official payment receipt, the collector must furnish the application reference number.
